ACL Reconstruction Recovery in South Africa
ACL reconstruction is only step one. The rehab that follows over the next nine to twelve months is what decides whether you return to sport safely, or reinjure the graft. This is what a structured recovery looks like.
Phase 1 (0 to 2 weeks): protect and settle
Swelling management, full extension range, quad activation, protected weight bearing according to the surgeon. Missing full extension in this phase creates long term problems, so work hard on it early.
Phase 2 (2 to 12 weeks): strength and control
Progressive loading, exercise bike, single leg strength work, balance and proprioception, gait retraining. Most patients are walking normally and back at desk work early in this phase.
Phase 3 (3 to 6 months): running and plyometrics
Structured return to running, jumping and landing mechanics, change of direction drills. This is where objective strength testing becomes non negotiable.
Phase 4 (6 to 12 months): return to sport
Sport specific training, contact drills if relevant, and clearance based on strength symmetry, hop testing and confidence. Rushing this phase is how graft re ruptures happen.

Weekly early on, then reducing to fortnightly and monthly as you progress.
