Tension Headaches and Neck Pain: Physio Answers
If your headaches start at the base of the skull, wrap around the head and worsen through the day, your neck is very likely part of the story. These are called cervicogenic headaches, and they respond remarkably well to physiotherapy.
How the neck causes headaches
The upper three cervical vertebrae share nerve connections with the areas that feel head pain. When these joints stiffen or the surrounding muscles tighten, the brain interprets some of that input as a headache. Fix the neck and, in many patients, the headaches fade with it.
Signs your headache is cervicogenic
Pain that starts in the neck or back of the head, is usually one sided, worsens with certain neck movements or sustained postures, and eases with rest or manual therapy. Migraines can co exist, so a proper assessment matters.
What physiotherapy involves
Precise mobilisation of the upper cervical joints, soft tissue release of the deep neck extensors, dry needling for trigger points in the upper trapezius and suboccipital muscles, and a strengthening programme for the deep neck flexors and scapular stabilisers.
Lifestyle habits that reduce frequency
Keep your screen at eye level, take regular movement breaks, hydrate properly, sleep on a supportive pillow, and manage stress. None of these fix the underlying stiffness on their own, but combined with treatment they often halve the frequency of your headaches.
