Tennis Elbow: A Physio's Recovery Guide
Tennis elbow rarely gets better with rest alone. Structured loading is the key. Here is how a physiotherapist actually treats this stubborn but very manageable problem.
What tennis elbow really is
A tendinopathy of the wrist extensor tendons where they attach to the outside of the elbow. It builds up gradually from repetitive gripping, lifting or typing, not usually from playing tennis.
Why rest alone does not work
Tendons need load to heal. Complete rest calms symptoms briefly but leaves the tendon just as weak as before, so pain returns as soon as you use the arm normally.
The rehab plan
Progressive isometric grip work, followed by heavy slow resistance exercises for the wrist extensors, and gradual reintroduction of aggravating activities. Manual therapy and dry needling can speed up the early phase.
Practical daily tips
Switch to a lighter kettle, carry shopping in both hands, adjust your mouse grip, use a counterforce brace during the day if it helps. These small changes buy the tendon room to heal.

Frequently asked questions
Do cortisone injections help?+
They give short term relief but often lead to recurrence and can weaken the tendon. Rehab beats them long term.
How long does recovery take?+
Most cases resolve in six to twelve weeks with structured loading.
Should I use a brace?+
A counterforce brace can help during high load activities, but it is not a treatment on its own.
