Sitting vs Standing Desks: What a Physio Thinks

11 July 2026 · By Nhlamulo Khosa, BSc Physiotherapy (UCT)

The best position is your next one. Standing desks help many patients, but they are not the whole answer. Here is what we actually recommend for desk workers with back and neck pain.

What standing desks do well

They break up long periods of sitting, activate the postural muscles, and reduce lumbar disc pressure compared with slumped sitting. For patients with disc related back pain, this is often meaningful.

Where they fall short

Standing all day just swaps one static posture for another. It also loads the feet, knees and low back in a new way, which can create fresh problems. Standing does not automatically fix bad screen height or a rounded upper back.

The best of both

Alternate between sitting and standing every 30 to 45 minutes. Use an anti fatigue mat when standing. Combine with brief movement breaks and two to three short walks across your workday.

What actually fixes the pain

A proper assessment, targeted strengthening and mobility work, and consistent habits. The desk setup is a supporting act, not the star of the show.

If you're in Centurion or Midrand and want personal guidance, our physiotherapist offers same day appointments in clinic in Eco Park, Centurion, or at your home.

Frequently asked questions

Should I buy a standing desk?+

If your work allows it and you will actually alternate, yes. But do not expect it to fix pain on its own.

How long should I stand at a time?+

Usually 20 to 40 minutes at a stretch, alternated with sitting.

Do I need special shoes?+

Supportive, cushioned footwear or an anti fatigue mat is helpful if you stand for long periods on hard floors.
